PER DIEM MSW SOCIAL WORKER

IMMEDIATE OPENING in our Northern office serving Essex, southern Franklin, and portions of St. Lawrence and Hamilton counties of New York State.

Working as part of an interdisciplinary team, you will serve people with terminal illnesses including the grief and loss experienced by families. You will also assist families with insurance and funding issues and making final arrangements. The right candidate will have:

• A current NYS MSW License
• Excellent skills and experience with group facilitation.
• Demonstrated experience working with families, responding to their emotional and psychological needs
• Working knowledge of North Country community networks of health and human services organizations
• Experience working with Medicare/Medicaid and/or health coverage issues.

RN CASE MANAGERS

Immediate openings for an RN Case Manager in our Northern and Southern offices. In addition to excellent verbal/written communication, the candidate will demonstrate experience in developing and maintaining rapport with patients, families and medical providers and the ability to work well with an interdisciplinary team.
This position includes a benefit package, mileage reimbursement, paid sick and vacation time, and 11 paid holidays.

Responsibilities and Duties
• Assesses and documents appropriateness for hospice at initial and ongoing assessment visit.
• Consults with Hospice Physician and/or clinical manager
• Provides information on hospice care and programs to referral sources and patients and families
• Completes written admission documentation with patients and families as needed
• Provides direct nursing care to patients in order to maintain the highest level of comfort and quality of life. Assists in directing and coordinating the physical care of the patient including providing medication, diet and other instructions as ordered by the physician, Primary Care Nurse, and Clinical Manager during telephone contacts or home visits.
• Seeks and obtains sufficient information necessary to function effectively in the Hospice setting where patient/family/caregiver require care.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Applies appropriate preventative and therapeutic nursing procedures and techniques and maintains communication regarding patient care with the physician and supervisory personnel.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given.
• Arranges for equipment and materials needed from community resources. Uses supplies and equipment effectively and efficiently.
• Teaches the patient/family/caregiver self-care techniques as appropriate. Recognizes and utilizes opportunities for health counseling with patients/families/caregivers. Works in conjunction with the other Hospice interdisciplinary team members.

SPIRITUAL CARE COORDINATOR

Immediate opening for a Spiritual Care Coordinator in our northern office serving Essex and southern Franklin Counties of New York.
The Spiritual Care Coordinator provides direct spiritual/pastoral support and/or counseling to patients/families, hospice staff, and volunteers as needed, acknowledging and respecting their beliefs, culture and values related to life's meaning, including suffering and loss.

Responsibilities and Duties
• Provides services related to the spiritual aspects of the plan of care including direct provision of service, support and monitoring of the progress of spiritual care referred to other local clergy.
• Participates in Interdisciplinary Team meetings.
• Maintains documentation of pastoral services in patient's medical records according to agency policy.
• Acquires on-going education and training through conferences, meetings, and current literature.
• Elicits and listens to story of patient, family and friends encouraging life review.
• Attends death of patient when needed, offering prayer and ritual as appropriate.
• Helps plan and conduct wakes, funeral and patient memorial services as circumstances warrant.
